We are excited to have you as a patient and want to make your visit as efficient as possible. If you have not done so already, please download, print and completely fill out forms 1-3 to bring with you for your first appointment. Please read the documents carefully and fill out as much as possible before your visit. This will speed up the time it takes to see the doctor(s), and will help us process your insurance and treatment.
A WORD ABOUT INSURANCE PLANS NOT PARTICIPATING WITH OUR OFFICE
Queen City Ear Nose and Throat accepts most major insurance plans. To confirm your in-network coverage with our practice, it is strongly recommended to contact your insurance carrier to confirm your plan's status with the our providers, including the detail of our office location, for your insurance plan coverage. The following insurance plan have different levels of coverage and may not be in-network depending on your specific plan; Blue Cross Blue Shield Blue Essentials, Blue Home or Blue Value. Additionally, if you reside outside North Carolina and have an HMO plan, these plans are likely to not cover visits and procedures within our offices.
Queen City ENT does not participate with South Carolina Medicaid plans.
